Director of Nursing / Nursing Supervisor / Clinical Manager

  • Director of Nursing – Oversees the clinical nursing operations of the agency, ensuring compliance with CMS regulations, supervising nursing staff, maintaining quality of care, and supporting the Administrator and Clinical Manager in coordinating patient services.
  • Clinical Manager – Provides oversight of all patient care services and personnel, including making assignments, coordinating care and referrals, and ensuring patient needs are continually assessed and plans of care are updated.

Skilled Professionals (Case Manager-RN, LPN, PT, OT, ST, MSW, HHA)

  • Skilled Nurse (RN/LPN) – Provides skilled nursing care, assessments, treatments, and coordination per the plan of care.
  • Physical Therapist (PT) – Evaluates mobility, delivers rehabilitative therapy, and completes required 30-day reassessments.
  • Occupational Therapist (OT) – Assesses ADL abilities, provides functional rehabilitation, and performs 30-day reassessments.
  • Speech Language Pathologist (SLP) – Evaluates and treats speech, language, cognitive, and swallowing disorders under the plan of care.
  • Home Health Aide (HHA) – Provides intermittent personal care and ADL assistance under RN or therapist supervision.
  • Medical Social Worker (MSW) – Addresses psychosocial needs through assessment, counseling, resource coordination, and care-plan support.

Administrative Support Services/Office Coordinators (Intake/Staffing, Therapy Coordinator, Health Information Management/Med Record, DME Coordinator)

  • Intake/Staffing Coordinator – Manages referrals, verifies orders, coordinates SOC/ROC scheduling, and serves as the communication hub for clinical operations.
  • Therapy Coordinator – Oversees PT/OT/SLP referrals, scheduling, visit frequency, supervision requirements, and therapy compliance.
  • Medical Records Personnel – Maintains complete, timely, CMS-compliant patient records, orders, documentation flow, and audit-ready charts.
  • DME Coordinator – Coordinates ordering, delivery, and documentation of durable medical equipment, ensuring medical necessity and vendor compliance.

Quality Assurance Team, Professional Advisory Committee

  • Quality Assurance Team – Monitors documentation, performance, and CMS compliance by reviewing records, tracking quality indicators, supporting QAPI, conducting internal audits, and collaborating with clinical and administrative staff to ensure safe, effective, standard quality care.
  • Professional Advisory Committee – Provides professional oversight to ensure CMS-compliant, high-quality care by reviewing agency policies, evaluating QAPI performance, assessing service standards, advising on community needs and interdisciplinary coordination, and meeting regularly with representation from key clinical disciplines.

Billing, Accounting

  • Billing – Manages the full billing cycle by submitting claims, verifying payer requirements, resolving edits or denials, posting payments, maintaining audit-ready records, and coordinating with clinical staff to ensure compliant, timely reimbursement.
  • Accounting – Oversees financial recordkeeping by managing payables/receivables, reconciling accounts, preparing financial reports, tracking budgets, ensuring cost-report compliance, and supporting audits and fiscal planning.

Community Liaison

Builds referral relationships, educates providers on home‑health services, supports intake coordination, tracks outreach trends, and represents the agency professionally to ensure smooth patient access and community engagement.
